Referrals to Southcity Clinic are usually made
through a GP or other healthcare professional in the drug and
alcohol field.
If you are unsure as to the appropriateness
of a referral you are welcome to contact the clinic and consult a
member of the clinical staff. Alternatively you may select the
pathways option (from the menu on the right
of the screen) to see which service is most appropriate for your
client.
The assessment process has two components:
1. An
initial interview with one of the counselling clinicians at the
Clinic followed by 2. A medical assessment with one of the
alcohol and drug medical specialists.
Where possible, Clinic
staff will endeavour to arrange these two appointments on the same
day however, due to time constraints, this is not always possible.
In some situations, the medical appointment will be on a subsequent
day to the initial assessment interview.
The referrer or the
patient may make the initial assessment appointment.
To meet funding requirements and assist in the
development of an appropriate treatment plan, it is most important
that a referral letter be provided to the Clinic either prior to or
at the commencement of the assessment interview.
If you are unsure as to the information
required in the referral please use the form provided
(select from the menu on the top right of this
page).
